Semaglutide dose ladder economics
Semaglutide's ladder has five steps and takes at least sixteen weeks to climb. What that costs depends entirely on whether your program charges by dose — and 8 of the 29 rankable programs have not published enough to let you work it out.
The five steps
The approved labelling for weight management titrates 0.25 mg, 0.5 mg, 1.0 mg, 1.7 mg and 2.4 mg per week, with about four weeks at each step. The first two are explicitly not treatment doses — they exist to let gastrointestinal side effects settle.
That means a minimum of eight weeks paying for a dose that is not yet doing the work, and about sixteen weeks before reaching the maximum. On a flat program those months cost the same as any other. On a tiered program they are cheaper, and the advertised price usually describes them.

The part the price tables leave out
Trials of the approved product found that stopping leads to substantial weight regain, which is why GLP-1 treatment is generally framed as long-term rather than a course you finish. That reframes the whole cost question: the relevant figure is not what a year costs but what an indefinite commitment costs, and a rate that cannot rise is worth more under that framing than under a one-year one.
It is also the strongest practical argument for reading cancellation terms as carefully as prices. A programme you may be on for years is one whose exit terms you will eventually use.
