Foundayo (orforglipron): the first oral small-molecule GLP-1
In April 2026 the FDA approved orforglipron, marketed as Foundayo — the first oral small-molecule, non-peptide GLP-1 receptor agonist approved for weight management. It delivers around 12% weight loss at the top dose, trailing the leading injectables, and can be taken at any time of day without the food and water restrictions that constrain oral semaglutide.
Why a small molecule changes the economics
Semaglutide and tirzepatide are peptides: expensive to manufacture, fragile, and poorly absorbed through the gut. That is why injectables dominate and why approved oral semaglutide needs an absorption enhancer and a strict empty-stomach protocol.
Orforglipron is a small molecule. It can be made by conventional chemical synthesis, at scale, in tablet form, with no cold chain. That is a structurally different cost base, and reporting on the federal pricing deal indicates the initial dose of future approved oral GLP-1s will be $150 a month on TrumpRx — a number that would sit at the bottom of our entire compounded field.
The trade-off is efficacy
Around 12% is meaningful and it is not what the top injectables deliver. The honest framing is a ladder: convenience and cost at one end, magnitude of effect at the other. Which is right is a clinical conversation, not a price comparison.
What it means for compounded programs
Compounded semaglutide floors at $129 a month all-in here, held by bmiMD. The compounded pitch is the same molecule, much cheaper, review removed. An approved oral competing on convenience and manufacturing cost attacks that from a direction compounding cannot copy — you cannot compound your way to a small molecule you have no approved supply of.
What we are not saying
We have not captured Foundayo pricing first-party, so it is not in our rankings and we quote no figure of our own for it. When we have a dated, verifiable capture it will appear with its evidence status like everything else.
Verified . Regulatory positions, prices and trial results move — check the primary source before relying on a date. Compounded tirzepatide and semaglutide are not FDA-approved products.
Questions
What is Foundayo?
The brand name for orforglipron, approved by the FDA in April 2026 as the first oral small-molecule, non-peptide GLP-1 receptor agonist for weight management. It can be taken at any time of day without food or water restrictions.
How much weight loss does orforglipron produce?
Around 12% at the top dose, which trails the leading injectable GLP-1s.
How much will oral GLP-1 pills cost?
Reporting on the federal pricing deal indicates the initial dose of future FDA-approved oral GLP-1s will be $150 a month on TrumpRx. We have not captured that price first-party, so it is not in our rankings.
